青少年突发性聋预后与一些因素的相关性探讨

目的探讨青少年突发聋的病因、临床特点、疗效及影响预后的因素。方法对1997-2002年的46例(52耳)20 岁以下青少年突发性聋的临床资料进行分析。结果男女患者构成比约为2.4:1,大部分的患者发病前有较明确的上呼吸道感染史和精神压力。初诊时两耳同时发病占13.04%,伴眩晕者占28.26%;0.25-4KHz频率平均听力损失中重度(>60dB)占53.85%,听力曲线全耷型占46.15%。总体疗效为痊愈21.15%,显效17,3l%,有效25%,总有效率63.47%。结论在影响青少年突发性耷预后的因素中,发病前有无感冒史及治疗中精神因素、初诊时听力损失程度、纯音听力曲线类型及开始治疗时间与预后有关,而性别、单双耳发病、有无眩晕,以及用药种类等因素对预后的影响不明显。基本符合文献报道的一般性规律,但亦有自身的特点。

Sudden deafness in juveniles: Study on the correlation factors with prognosis

Objective To study the cause, clinical characteristics, treatment, efficacy and the main factors affetcting the prognosis of sudden deafness in juveniles. Methods Clinical data of forty - six cases(52 ears) about juvenile wi(h sudden deafness were reviewed between 1997 and 2002. Results The number of male cases is as about 2. 4 times as that of female cases, nearly the more cases had history of virus infection and nerves. A/nong them 13.04% were simultaneous bilateral ears, 28.26% vertigo, 53.85% the serious hearing theshold levels of both speech and high frequencies, 46. 15% the over type of primary audiogram. The total effective rate was 63. 47%. Conclusion The correlative factors affetcting the prognosis of sudden deafness in juveniles. The prognosis is related with the history of virus infection, the hearing threshold levels of both speech and high frequencies, the type of primary audiogram, the time of beginning treatment, but not with the sex, singular or bilateral, the vertigo and the medicine of the treatment. Some aspects of sudden deafness in juvenile were similar to that in adult, there are also some special characteristics about it.
