布托啡诺复合罗哌卡因用于产科硬膜外镇痛的观察

摘要] 目的：比较布托啡诺复合丙泊酚及单用丙泊酚用于人工流产术的效果及不良反应，探讨布托啡诺应用的安全性及有效性。方法：择期行人工流产术患者40 例，随机分为两组：布托啡诺-丙泊酚组（BP组）和丙泊酚组（P组），每组20例。观察两组患者围麻醉期血流动力学及SpO2变化、丙泊酚用量、诱导时间、苏醒时间、恢复正常行走时间、术中及术后镇痛情况，记录术中呼吸抑制、术后嗜睡、恶心呕吐等相关并发症、术中体动反应、术后宫缩痛及患者满意度情况。结果：P组术中MAP显著低于BP组水平（P＜0.01）。BP组丙泊酚用量与P组相比无显著性差异，但诱导时间显著短于P组（P＜0.01）。BP组苏醒时间及恢复正常行走时间较P组有所延长，但无统计学意义。BP组术中及术后镇痛效果均明显优于P组（P＜0.01），丙泊酚注射痛、体动、宫缩痛发生率均明显低于P组，两组呼吸抑制发生率无显著性差异。结论：布托啡诺复合丙泊酚用于人工流产术具有麻醉效果好、麻醉并发症少的优点，可安全用于人工流产术的麻醉，值得临床推广应用。

Anesthesia with butorphanol combined with propofol for painless artificial abortion

【Abstract】 Objective:To observe the clinical and side effect of premedication with butorphanol in propofol anesthesia undergoing painless artificial abortion. Methods: 40 patients undergoing artificial abortion were randomly divided into butorphanol combined with propofol(group BP,n=20) and propofol alone (group P,n=20) before surgery. The dynamics perioperation, pulse oxygen saturation, dosage of propofol, induced time, recovery time, leaving time, analgesic effect in operation and postoperation, respiratory depression, drowsiness, nausea , vomiting, movement in operation, uterine contraction pain postoperation and the satisfication of the patients were observed. Results: MAP during operation were significantly lower in group P than those in group BP(P<0.01). The dosage of propofol, recovery time, leaving time and respiratory depression were no significant difference between group BP and P(P>0.05).Induced time in group BP was significantly shorter than in group P(P <0.01).There were better analgesic effect ,less pain during injection of propofol, less operative movement and less uterine contraction pain in group BP compared with in group P(P <0.01).Conclusions: Perfective anesthetic effect and less complications were the main advantages of butorphanol combined with propofol for painless artificial abortion and were available for the clinics.
