线粒体脑肌病的MRI表现及诊断价值

目的探讨线粒体脑肌病的MRI表现特征及其诊断价值。方法收集经MRI检查、实验室检查及临床和病理证实的线粒体脑肌病9例,对其脑部MRI表现进行回顾性分析。结果 9例线粒体脑肌病的脑部病灶在T1WI均呈低信号、T2WI呈高信号,病变主要累及颞叶、顶枕叶皮层及皮层下白质部,甚至基底节区,大部分呈对称分布,可不对称,有多发性、迁移游走多变性和与血管分布不一致性,仔细观察呈水印样,可能与脑组织层状坏死有关。结论线粒体脑肌病的脑部MRI表现具有特征性,特别是MRI特殊序列（DWI、FLAIR、MRS）在线粒体脑肌病的鉴别诊断及组织代谢检测方面具有重要的价值和前景。

MRI Manifestations and Diagnostic Value of Mitochondrial Encephalomyopathy

Objective To invegtigate the MRI manifestations and diagnostic value of mitochondrial encephalomyopathy.Methods 9 cases with mitochondrial encephalo-myopathy were retrospectively analyzed which was validated by MRI pathological analysis and biochemical laboratory examinations.Results The 9 cases mitochondrial encephalomyopathy in the brain showed low signal intensity on T1WI and high signal intensity on T2WI,the lesions mainly involved the cortical layer and subcortical white matte of the temporalparietal-occipital lobe,even the basal ganglia.most lesions were symmetrical,can be asymmetric,with multiple,migration and vascular distribution of variability and inconsistency,watermarks samples were carefully observed,may be layered with brain tissue necrosis.Conclusion mitochondrial encephalomyopathy with characteristic brain MRI findings,in particular special MRI sequences(DWI,FLAIR,MRS)in the differential diagnosis of mitochondrial encephalomyopathy and metabolic testing has an improtent organizatinal value and prospects.
