16层螺旋CT检测左心房血栓的临床价值研究

目的研究16层螺旋CT（MSCT）在左房血栓检测方面的临床价值。方法对60例患者（37例心房颤动患者,23例二尖瓣狭窄患者）分别进行16层螺旋CT及经食管超声检查,以经食管超声心动图为金标准,对16层螺旋CT检出左心耳血栓情况进行评价。结果 MSCT检测出7例左房血栓,其中4例TEE确认为左房血栓,另外3例TEE确认为假阳性。MSCT诊断左房血栓的敏感性、特异性、阳性预测值和阴性预测值分别为100.0%、95.4%、70.0%、100.0%。结论 MSCT对检测左房血栓有很高的敏感性,可以作为左心房血栓筛选的影像学方法。

The Research of the Clinical Value of 16-slice Spiral CT in Detecting the Left-atrium Thrombus

Objective To study the clinical value of 16-slice spiral CT in detecting the leftatrium thrombus.Methods 37 cases of atrial fibrillation patients and 24 cases of mitral valve stenosis received 16-slice spiral CT and TEE(trans-esophageal echocardiography) examination.The TEE results were regarded as gold standard while detecting the leftatrium thrombus by using 16-slice spiral CT.Results 16-slice spiral CT have detected 7 left-atrium thrombus patients while only four of them were diagnosed by TEE.The sensitivity,specificity,positive and negative predictive value for MSCT were 100.0%,94.6%,57.1%,100.0%,respectively.Conclusion MSCT is exquisitely sensitive for the detecting LAT,and may served as an alternative imaging modality for the evaluation of the left atrial thrombus in patients with AF or mitral valve stenosis.
