多层螺旋CT血管造影(MSCTA)诊断胃肠道间质瘤的价值

摘    要:目的探讨多层螺旋CT血管造影(MSCTA)对胃肠道间质瘤(GIST)的诊断价值。方法搜集南方医科大学附属南海医院2009年7月-2011年6月间经手术及病理证实的胃肠道间质瘤(GIST)患者18例,采用最大密度投影(MIP)、多平面重建(MPR)及容积再现(VR)血管添加技术(AV)显示肿瘤及其供血动脉,从而判断肿瘤的起源,并与手术结果对照。结果 18例GIST患者中,肿瘤起源于胃8例,空肠2例,回肠5例,结肠2例,1例来源于肠壁或肠系膜,未能明确其具体起源。MSCTA均清晰显示肿瘤主要供血动脉,显示率100%(18/18),定位诊断准确度为94.4%(17/18)。结论多层螺旋血管造影(MSCTA)诊断胃肠道间质瘤的准确率高,显示肿瘤供血动脉是判断GIST起源的主要依据,有重大临床应用价值。

Diagnostic Value of MSCTA in Gastrointes-tinal Stromal Tumor

Abstract:Objective To investigate the value of multi-slice spiral CT angiography(MSCTA) in diagnosing gastrointestinal stromal tumor(GIST).Methods Eighteen patients of GIST confirmed by operation and pathology were retrospectively investigated from July 2009 to June 2011 in our hospital.All these patients accepted enhanced CT scanning and vascular three-dimensional reconstruction with maximum intensity projection(MIP),multiplanar reconstruction(MPR) and volume rendering(VR) based on add vessel(AV),The tumor and the feeding arterys were show,so the origin of tumor was pre-judgment.All of these were compared with surgical results.Results All these 18 cases of GIST,8 cases of tumor originated from stomach,2 jejunum,5 llnum,2 colon and 1 intestinal wall or mesentery but unknown the exact location.The accuracy rate was 94.4%(17/18) according to the feeding artery which displaying rate was 100%(18/18).Conclusion The feeding artery on MSCTA is an important evidence judging the origin of GIST,with a relative high accuracy.
