135例甲状腺微小癌临床诊治分析

目的]探讨甲状腺微小癌的临床特点、诊断和外科治疗方式。方法]回顾分析浙江省台州医院1987年6月至2002年6月手术治疗的135例甲状腺微小癌的临床资料。结果]135例甲状腺微小癌中76例(56．30％)术前拟诊甲状腺良性病变,而术中或术后发现为甲状腺微小癌。对原发灶均行一侧腺叶及以上切除。37例行功能性颈淋巴清扫中有13例有颈淋巴结转移。123例(91．11%）随访6个月至10年,无1例死亡。结论]甲状腺微小癌多数因拟诊甲状腺良性病变在术中或术后发现,腺叶切除可作为主要手术方式,对临床淋巴结肿大者行同侧功能性颈淋巴结清扫术。

Clinical Analysis of 135 Cases of Thyroid Microcarcinoma

To investigate the clinical characteristics,diagnosis and surgical treatment of thyroid microcarcinoma.One hundred and thirty-five cases of thyroid microcarcinoma from June1987to June2002in Taizhou hospital,Zhejiang province were retrospectively analyzed.Seventy-six cases with thyroid microcarcinomas(56.30%)were found incidently during or af-ter operation for"benign thyroid disease".All patients underwent thyroid lobectomy or more.Homo-lateral functional neck dissection was taken on37cases and neck lymph node mastastasis was found in13cases.One hundred and twenty-three patients(91.11%)were followed up for6months to10years.No patient died.Conclusion]Most thyroid microcarcinoma were found during or after oper-ation for"benign thyroid disease".Thyroid lobectomy is the main surgical principle.Homo-lateral functional neck dissection should be taken for patients with neck lymph node mastastasis.
