高血压脑病影像学特征分析

目的分析高血压脑病的影像学特征,探讨其影像学诊断和鉴别诊断。方法回顾性分析35例临床确诊为高血压脑病患者的头部CT、MRI表现及临床资料(侧重影像学)。结果 35例患者均行头颅CT检查,表现阳性12例,阴性23例;35例中18例同时行MRI检查,12例表现阳性,6例表现为阴性。阳性病例影像学主要表现为脑水肿,伴灶状脑出血6例。患者收缩压为180～230mmHg、舒张压为120~150mmHg,均伴有不同程度颅内压升高临床表现。结论高血压脑病的影像学改变主要为脑水肿表现,其诊断与鉴别需密切结合临床。

Characteristics of imaging in patients with hypertensive encephalopathy (an analysis of 35 case)

Objective To enhance the levels of diagnosis and differential diagnosis in the patients with hypertensive encephalopathy.Method The clinical data including the imaging data of 35 patients with hypertensive encephalopathy were analyzed retrospectively.Results There were positive findings in 12 patients and not in 18 patients on CT.The MRI examination was performed in only 18 patients.There were positive findings in 12 patients and not in 6 patients on MRI.The positive findings mainly included brain edema and focal cerebral hemorrhage.The systolic blood pressure ranged from 180 to 230 mmHg and diastolic blood pressure from 120 to150 mmHg in the patients.There were the clinical manifestations of high intracranial pressure in all the patients.Conclusions The main manifestation of imaging is cerebral edema,and the diagnosis and differential diagnosis may be made by imagings combined with clinicalm anifestations in the patients with hypertensive encephalopathy.
