手指抠挖致视神经撕脱伤临床分析

目的报告家庭暴力事件中由手指抠挖所致的视神经撕脱伤(ONA),眼球脱出,视力严重受损或丧失。方法回顾我院1997年11月-2006年3月收治3例视神经撕脱伤视功能严重受损病例。分析外伤原因、就诊时的症状和体征、诊断和治疗、以及结果。结果本组病例均为女性,平均年龄35岁。受伤均为双方发生纠纷时丈夫施虐所致。就诊时视力有1例为光感,2例无光感,1例眼球丢失。所有病例都有ONA。3例均因要求美容而接受了眼球摘除术。结论手指抠挖致ONA临床罕见,预后差,对家庭和社会造成严重危害,应为全社会共同关注。

Clinical analysis of optic nerve avulsion from finger

Objective To report severe visual loss caused by optic nerve avulsion(ONA) and eye globes dislocation from finger to pull in domestic violence cases.Methods A retrospective review of medical records of all 3 patients with optic nerve avulsion who sustained severe visual loss,from November 1997 to March 2006.The data were analysed for the cause of visual loss,presenting symptoms and signs,diagnostic studies and treatments,and final outcome.Results There were three females with an average age of 35 years,injured by her husbands.Presenting visual acuity(VA) was light perception(LP) in one patient and no light perception(NLP) in tow.All the 3 patients had evidence of ONA.Three eyes required enucleation for painful blind eye or to achieve optimal cosmesis.Conclusion The optic nerve avulsion from finger is a rare but serious injury and the prognosis is usually poor.Domestic violence incident on the family and society causing serious harm,should be a common concern throughout society.
