食管癌螺旋CT征象与Cox-2、nm23 表达间关系的研究

摘    要:目的探讨食管癌螺旋CT(SCT)征象与癌组织中Cox-2、nm23表达间的关系。方法44例食管癌患者术前均行SCT扫描,并与手术病理对照,术后标本用免疫组织化学方法检测癌组织中Cox-2、nm23表达情况,用SPSS12.0软件进行对照分析。结果(1)SCT对食管癌有无邻近结构受侵和有无淋巴结转移判断准确率分别为88.6%(39/44)和90.9%(40/44),与手术病理对照一致性好(P<0.0001);(2)Cox-2阳性表达率79.5%(35/44),与SCT征象上癌肿长度、邻近结构侵犯、浸润深度、淋巴结转移呈正相关(P<0.05);(3)nm23阳性表达率68.2%(30/44),与SCT征象上浸润深度、淋巴结转移呈负相关(P<0.05)。(4)Cox-2阳性表达和nm23低表达共同促进食管癌的淋巴结转移。结论SCT扫描能较全面地显示食管癌影像学特征,SCT征象与Cox-2、nm23异常表达关系密切。

Abstract:Objective To evaluate the correlation between spiral CT ( SCT ) features and expression of cyclooxygenase-2 , nm23 in esophageal carcinoma.Methods Spiral CT scans were preformed in 44 patients with esophageal carcinoma before operation, and the results were compared with that of operation and pathology. Immunohistochemical streptavidin peroxidase conjugate method was used to analyze the expression of Cox-2 and nm23 in esophageal carcinoma. Statistical analysis was performed with SPSS 12.0 software. Results (1) The accuracy of spiral CT scan in judging adjacent- infiltration and lymph node metastasis in 44 patients was 88.6%(39 of 44) and 90.9% (40 of 44) respectively,there was significant coincidence with operation and pathology(P<0.0001); (2) The positive rate of Cox-2 was 79.5% (35 of 44), which was positively in relation to SCT features including tumor size, adjacent-infiltration , infiltration depth and lymph node metastasis(P<0.05);(3) The positive rate of nm23 was 68.2% (30 of 44),which was negatively in relation to SCT finding including lymph node metastasis and infiltration depth(P<0.05);(4)The abnormal expression of Cox-2 and nm23 protein might act on the lymph node metastasis of esophageal carcinoma together.Conclusion SCT scan can well display the imaging feature of esophageal carcinoma. SCT feature has a close correlation with expression of Cox-2, nm23.
