口腔鳞癌浸润前沿中基质金属蛋白酶-9及其抑制剂-1的表达

摘    要:目的:检测基质金属蛋白酶9(MMP9)及其抑制剂1(TIMP1)在口腔鳞状细胞癌及浸润前沿中的表达并探讨其意义。方法:54例标本行WHO分级和IFG计分,应用免疫组化SP法检测54例口腔鳞癌组织中MMP9和TIMP1的表达。结果:MMP9和TIMP1蛋白在肿瘤细胞和间质细胞均有表达。MMP9阳性表达随浸润深度、TNM分期、WHO病理分级和IFG计分递增而递增(P<0.01),与浸润深度、TNM分期和IFG计分呈正相关(相关系数分别为rs=0.420,rs=0.460和rs=0.361,P<0.05;Spearman检验)。TIMP1随细胞分化程度增加呈递减趋势,但无统计学意义;与IFG计分有显著差异(P<0.01)。口腔癌浸润前沿肿瘤细胞表达与癌中心细胞表达有显著性差异(P<0.01)。结论:MMP9和TIMP1在口腔鳞癌的上皮及间质细胞中均有不同程度的表达;在浸润前沿癌细胞的表达明显强于中心区,表明肿瘤浸润前沿细胞侵袭性更强。

Abstract:Objective: To investigate the expression of matrix metalloproteinase-9(MMP-9) and tissue inhibitor of metalloproteinase-1(TIMP-1) and to explore its significance in oral squamous cell carcinoma (OSSC). Methods: The expressions of MMP-9 and TIMP-1 in 54 cases of OSCC were detected by immunohistochemical SP method. Results: MMP-9 and TIMP-1 were mainly expressed in the cytoplasm in tumor cells, and rarely in tumor stroma. The relation of the expression of MMP-9 to the depth of invasion, TNM stage, WHO grading and IFG was positive,and was associated with depth of invasion, TNM stage and IFG positively(r s=0.420,r s=0.460 and r s= 0.361,respectively, P<0.01). The expression of TIMP-1 had a tendency to be associated negatively with a lower cell differentiation, but the association had no statistical significance. However, the relation to IFG was positive (P<0.01). Positive rate of expression of MMP-9 and TIMP-1 in the invasive front was significantly higher than that in the central region of OSCC (P< 0.01). Conclusion: The intense of expression of MMP-9 and TIMP-1 was observed in tumor cells and stroma cells differently;its expression was significantly stronger in the invasive front than that in the center of tumor, which indicated that tumor cells in invasive Matrix Metalloproteinase front has stronger potentialities of invasion in OSCC.
