俯卧位辅助用具在视网膜脱离手术后应用的效果观察

目的观察新月形面枕和u形床垫在视网膜脱离眼内填充术后俯卧位患者的应用效果。方法88例视网膜脱离硅油眼内填充术后患者随机分为常规俯卧位组（对照组）和使用新月形面枕和u形床垫组（试验组），观察两组患者术后体位维持时间、身体不适、眼部不适及术后并发症。结果试验组患者较对照组患者体位维持最长时间、体位维持平均时间、睡眠时间均明显延长，差异均有统计学意义（P〈0．05），颈部不适、胸闷、腰部不适、窒息感、眼球下坠感、眼部疼痛感、术后青光眼的发生率均明显降低，差异均有统计学意义（P〈0．05）。结论新月形面枕和U形床垫在视网膜脱离眼内填充术后俯卧位的应用中取得良好效果，具有方便、实用及减少并发症等优点。

An observation concerning the effect of the application of prone position maintenance device in pa- tients after retinal detachment operation

Objective To observe the effect of the application of crescent-shaped face pillow and U-shaped cushion to help patients to maintain prone position after retinal detachment operation with silicone oil tamponade. Methods Eighty-eight consecutive patients with retinal detachment after silicone oil tamponade were recruited and separated into two groups randomly： the study group and the control group. Patients in the study group used the crescent-shaped face pillow and U-shaped cushion and while patients in the control group maintained prone position without medical protective device. The duration of position maintenance, physical discomforts, eye discomforts and postoperative diseases were analyzed. Results Compared with the control group, patients in the study group hold prone posture significantly longer, including the max- imum postural maintain time and the mean postural maintain time, and slept time significantly longer （ P 〈 0.05）. Patients in the study group also reported significantly less neck discomfort, chest tightness, lumbar discomfort, choking sensation, heavy sensation of the eye, and eye pain than the control group （ P 〈 0.05 ）. Postoperative glaucoma was found less in the study group in comparison with the control group （P 〈 0.05 ）. Conclusion The crescent-shaped face pillow and U-shaped cushion are a good position maintenance device to help patients after retinal detachment operation with silicone oil tamponade to keep prone posture.
