剥脱性食管炎18例诊治分析

目的探讨剥脱性食管炎患者的临床特点及内镜表现,提高对本病的认识水平。方法回顾性分析1997年1月至2011年10月诊治的剥脱性食管炎患者临床及内镜检查资料。结果 18例剥脱性食管炎,均经保守治疗痊愈,年龄19～63岁,平均年龄38.9岁,有明确吞食干硬粗糙物史者13例。吞咽疼痛18例,呕血8例,胸痛9例。食管黏膜缺损5例,黏膜剥脱12例,黏膜下血肿8例。结论剥脱性食管炎患者多有吞食干硬粗糙物史,以吞咽疼痛,呕血及胸骨后疼痛多见。内镜检查有利于及早确诊及治疗,表现为食管黏膜剥脱、黏膜下血肿、黏膜缺损、出血,具有特征性表现。

Exfoliative Esophagitis Diagnosis and Treatment of 18 Cases

Objective To investigate the exfoliative esophagitis in patients with clinical characteristics and endoscopic findings, raise the level of awareness of the disease. Methods A retrospective analysis in January 1997 - October 2011 exfoliative esophagitis diagnosis and treatment of clinical and endoscopic examination data. Results 18 cases of exfoliative esophagitis were cured by conservative treatment, aged 19 to 63 years, mean age 38.9 years, with a clear history of swallowing hard, dry rough material in 13 cases. Pain on swallowing 18 cases, 8 cases vomiting, chest pain in 9 cases. 5 cases of esophageal mucosal defects, mucosal stripping 12 cases, 8 cases of submucosal hematoma. Conclusion Exfoliative esophagitis patients have swallowed hard, dry rough material history to swallowing pain, vomiting and chest pain more common. Endoscopy favor early diagnosis and treatment, the performance of the esophageal mucosa exfoliation, submucosal hematoma, mucosal defects, bleeding, with a characteristic performance.
