脊髓造影联合CTM检查在退行性腰椎侧凸性椎管狭窄中的诊断价值研究

目的探讨研究脊髓造影联合CTM在退行性腰椎侧凸性椎管狭窄中的影像学诊断价值。方法选取具有典型临床特征并经手术证实为退行性腰椎侧凸性椎管狭窄的患者50例,术前分别给予脊髓造影、CTM检查和MRI检查,进行影像学分析,观察其病变部位和程度,比较影像学诊断符合率。结果脊髓造影、CTM检查、脊髓造影联合CTM检查和MRI检查大多能显示出该患者具有退行性腰椎管狭窄的典型表现,与手术符合率分别为为90%、94%、98%和86%;脊髓造影和CTM的诊断符合率与MRI相比较,P<0.01差异具有统计学意义,均优于MRI;脊髓造影联合CTM的诊断符合率与MRI相比较,P<0.05,差异显著具有统计学意义,明显优于MRI。结论脊髓造影联合CTM应用于退行性腰椎侧凸性椎管狭窄的影像学诊断中,具有诊断符合率高、不良反应率低、有较高的诊断正确率等优点,明显优于MRI检查,值得临床应用和推广。

The Study of the Diagnostic Value of the of Myelography Joint the CTM Examination in Degenerative Lumbar Scoliosis Spinal Stenosis

Ohjective To explore myelography combined CTM Diagnostic value of imaging in degenerative lumbar scoliosis spinal stenosis. Methods To select with the typical clinical features and confirmed by surgery for degenerative lumbar scoliosis spinal stenosis patients 50 cases were given preoperative myelography, the CTM examination and MRI imaging analysis to observe the lesion and degree of comparative imaging diagnosis rate. Results most of the myelography, CTM examination, myelography combined CTM cheek and MRI examination showed a typical manifestation of the patients with degenerative lumbar spinal stenosis, surgery coincidence rate were 90%, 94%, 98% and 86 %; coincidence rate of diagnosis of myelography and CTM and MRI comparison, P〈0.01 difference was statistically significant, are superior to MRI; the myelography joint CTM diagnosis coincidence rate compared with MRI, P〈0.05, significant difference statistically significance, and significantly better than MRI. Conclusion myelography combined CTM for degenerative lumbar seoliosis spinal stenosis diagnostic imaging with the diagnosis rate, low rate of adverse reactions higher diagnostic accuracy, etc., significantly better than MRI, worthy of clinical application and promotion.
