| 摘 要： | 目的 探讨无创通气在重度支气管哮喘急性发作治疗时,对pH值、PaO_2、PaCO_2及一秒钟用力呼气量占预计值百分比(FEV_1%pred)、一秒钟用力呼气量占用力肺活量比值(FEV_1/FVC%)、最大呼气流量(PEF)的影响.方法 选择100例明确诊断为重度支气管哮喘急性发作患者,随机分为对照组与观察组,各50例,对照组给予常规治疗,观察组在常规治疗基础上给予经口鼻面罩的双水平气道正压(BiPAP)通气治疗.观察两组患者治疗前、治疗72 h后动脉血气分析(pH值、PaO_2和PaCO_2)的变化及治疗7 d后肺功能(FEV1%pred、FEV_1/FVC%和PEF)的改变情况.结果 观察组治疗72 h后pH值、PaO_2 值升高和PaCO_2值降低,较对照组差异有统计学意义(P＜0.05).观察组在治疗7 d后FEV_1%pred、FEV_1/FVC%和PEF值的提高值较对照组差异有统计学意义(P＜0.05).结论 无创通气在重度支气管哮喘急性发作治疗中可改善血气分析及肺功能.
