改良式手术治疗慢性硬脑膜下血肿

目的探讨馒性硬脑膜下血肿新术式的治疗效果．方法４２例慢性硬脑膜下血肿患者，根据血肿量的不同．于血肿侧钻１～２个孔，扩大骨窗至２．５ｃｍｘ２．５８ｃｍ，尽可能把术野中血肿的内膜层撕破、去除之，使脑脊液和血肿腔相沟通。结果采用改良式手术治疗４２例病人引流３ｄ，ＣＴ复查，１９例血肿完全消失（４５．２％），２３例血肿腔明显缩小（５４．８％）．结论颅骨钻孔扩大骨窗加血肿内膜剥脱法治疗慢性硬膜下血肿，疗效优于传统疗法．

Modified operahon style in treatment of chronic subduralhemotona

Objective To research a new operation method ror chronic subdural hemotoma. Method According to the size of hemotoma, the bone windows no the cramal bone of hematomale side were drilled and enlarged, then the intimal lager ofhematomale cavities were teared and removed to drained the blood of hernatomale cavities to cerebrospinal fluid. Result in all fourly-two patients who bed been operated with the new method, hemotomoale cavities reduced in 23 Patients (54.8 %) and disappeared in 19 patients (45.2% ) by CT re-examined after draining 3 days. Conclusion The therapeutic effect ofdrilling and enlarged the bone window of crinial bone plus intimal layer is better than the orthe traditional operation method for chronic subdural hemotoma.
