缝匠肌骨瓣转移治疗股骨头缺血性坏死

目的探讨股骨头缺血性坏死的外科治疗方法．方法３５髋３３例Ⅲ－Ⅳ期股骨头缺血性坏死患者，行坏死骨清除、减压、髋骨骨髓骨及火柴棒状骨加缝匠肌骨瓣移植手术治疗。结果３５髋３３例Ⅲ－Ⅳ期股骨头缺血性坏死患者，经缝医肌骨瓣转移手术治疗后，１．５ａ～５ａ（平均３．ｌａ）随访的有２７例２８髋。按照王义生的４级１２办法标准，结果优１５髋；良１１髋；可２髋；差０髋．优良率为９３％．结论缝匠肌辨转转移术具有（１）彻底清除坏死骨和减压，恢复股骨头的血流动力学平衡；（２）缝匠肌分辨大，血运丰富，能加速骨重建。适宜于年轻的Ⅲ－Ⅳ期股骨头缺血性坏死病人。

Sartorius muscle pedrcled bone graff for the surgical treatment of avascular necrotic of femoral head

Obicctive To search for surgical treatment of avascular mecrosis the femoralhead (ANFH).Methods A total of 35 hips in 33 adult patients with stag (3～4) were received complete removel of the necrotic bone, decreasing traosseus pressure and using illiae marrow bone, illiae bone chips, andthe muscle pedicled bone groft.Resulf A total of 35 hips in 33 sartorios adult patients with stage(3～4)were so treated; 28 hips in 27 patients were followed up for 1 .5～syears with an average of 3. 1 years. Accoding to WANG Yi-heng's standand: 15 hips were excellent; 11 hips were good; 2hips were general.The rate of the excellent and good results was 93 %. Conclusion The advantage of the sartorias muscle pedrcled bone is: (1) comptete removel of the necrotic bone and depression can recover the balance of hemodgnamics in the femoral head, (2) the big sartorius muscle pedicled bone graft with its sufficient blood supply is beneficial for new bone formation in femoral head.The authors believe that the technic is suitable for the treatment of ANFH of the stage 3～4 in young patients.
