胰岛素泵输入胰岛素治疗老年糖尿病58例临床观察

目的]观察采用胰岛素泵输入胰岛素治疗老年糖尿病的临床疗效。方法]符合病例入选标准的172例老年糖尿病患者按就诊先后顺序随机分为观察组、对照组,每组86例。2组患者均给予饮食控制、运动等常规治疗,同时2组患者均给予诺和灵R治疗。观察组采用胰岛素泵输入治疗,对照组采用诺和灵R早、午、晚、餐前皮下注射治疗。2组均以治疗1周为1个疗程,治疗2个疗程后观察2组血浆血糖和胰岛素水平、血糖达标时间,胰岛素用量及低血糖反应发生率,并根据上述指标的变化情况综合评价疗效。结果]空腹血糖、餐后2h血糖2组治疗前后组内相比差异有统计学意义(P﹤0.05),治疗后组间相比差异亦有统计学意义(P﹤0.05)。2组胰岛素日用量、血糖控制达标时间、低血糖发生情况相比差异有统计学意义(P﹤0.05)。观察组显效率为75.58%,总有效率为97.67%;对照组显效率为52.23%,总有效率为75.58%。2组显效率、总有效率相比差异有统计学意义(P﹤0.05)。结论]采用胰岛素泵输入胰岛素治疗老年糖尿病患者,平稳的控制血糖,降低低血糖反应的发生率,减少胰岛素用量。

CLINICAL OBSERVATION OF INSULIN PUMP IN TREATMENT OF 58 ELDERLY DIABETIC

Objective]To investigate the clinical efficacy of the use of insulin pump therapy in elderly diabetic.Methods]According to treatment order,172 diabetic patients were randomly divided into two groups,86 cases in each group.The patients in two groups were given diet,exercise and other conventional treatment,while those patients were treated with Novolin R treatment.Patients in the observation group were treated with insulin pump therapy,the control group was treated with hypodermic injection of Novolin R at morning,afternoon,evening,before meal.After 2 weeks of treatment(2 courses),observed the plasma glucose,insulin levels,time of blood glucose reach to standard,insulin dosage and incidence of hypoglycemia,and in accordance with the above indictors,comprehensively evaluated the efficacy.Results]The fasting plasma glucose,2-hour postprandial blood glucose in two groups before and after treatment were statistically significant(P﹤0.05).Daily dose of insulin,time of blood glucose reach to standard,incidence of hypoglycemia in two groups were statistically significant(P﹤0.05).The markedly effective rate was 75.58% in the observation group,total effective rate was 97.67%;in the control group,markedly effective rate was 52.23%,total effective rate was 75.58%.The markedly effective rate,total effective rate were statistically significant(P﹤0.05).Conclusion]Insulin pump in treatment for elderly patients with diabetes smoothly controls the blood sugar,reduce the incidence of hypoglycemia and the dosage of insulin.
