78例胎传梅毒血清学检测分析

目的]探讨新生儿胎传梅毒的血清学检测特点。方法]对78例疑为胎传梅毒新生儿(其母亲已在怀孕时被确诊梅毒)于出生时及6个月时采用甲苯胺红不加热血清实验(TRUST)、梅毒螺旋体颗粒凝集实验(TPPA)和抗体捕捉酶免疫测定技术(Tp-IgM-Cap-ELISA)进行血清学检测,并进行分组比较(Tp-IgM阳性者为A组,阴性者为B组)。结果]78例新生儿在出生时检测TRUST、TPPA均为阳性,Tp-IgM抗体阳性33例为A组,经过治疗6个月后TPPA仍均为阳性,Tp-IgM抗体阳性率为24.2%,TRUST阳性率为84.8%;Tp-IgM抗体阴性45例为B组,未经治疗6个月后仅1例幼儿TPPA及TRUST仍为阳性。结论]TRUST、TPPA阳性不能作为新生儿胎传梅毒的诊断,Tp-IgM抗体阳性者可确诊并需治疗,Tp-IgM抗体阴性者无需治疗TPPA及TRUST也可转阴。

SEROLOGICAL DETECTION AND ANALYSIS OF 78 CASES OF NEONATAL CONGENITAL SYPHILIS

Objective]To investigate the serological detection characteristic of neonatal congenital syphilis.Methods]78 cases of uncertain neonatal congenital syphilis,whose mothers were diagnosed congenital syphilis at pregnant stages,were tested by TRUST,TPPA,Tp-IgM-Cap-ELISA and were compared by group.The cases Tp-IgM-Cap-ELISA were positive in group A and the others in group B.Results]In all 78 newborns,the TRUST and TPPA were positive.Of them,the Tp-IgM of the 33 cases was positive in group A.After therapy for 6 months,33 cases TPPA were positive.The TRUST of the 28 cases(84.85%)was positive and the Tp-IgM of the 8 cases(24.2%)was positive.The Tp-IgM 45 of the cases was negative in group B.After 6 months,the TPPA and TRUST of only one case were positive without therapy.Conclusion]TRUST and TPPA could not be used as confirmatory tests but Tp-IgM.The Tp-IgM-Cap-ELISA of the cases was negative,and did not need treatment,and TRUST and TPPA can change to be negative too.
