2型糖尿病患者血清IL-18水平的检测和临床意义

[目的]观察2型糖尿病患者血清IL-18水平的变化,并探讨其临床意义。[方法]临床选择研究2型糖尿病病人67例,分为单纯糖尿病组(DM组,n=37例)和糖尿病肾病组(DN组,n=30例),并以正常健康人30例为正常对照组。所有研究对象均测定空腹血糖(FPG)、糖化血红蛋白(HbA1c)、24h尿微量白蛋白定量(24h-UA)和血清IL-18水平。[结果]DM组和DN组患者的的FPG、HbA1c和IL-18水平均较正常对照组明显升高(P﹤0.05);DM组和DN组对比,FPG、HbA1c无明显差异,但是DN组患者的24h-UA和IL-18水平明显高于DM组(P﹤0.05)。糖尿病肾病患者的IL-18水平与24h-UA有良好的正相关性(r=0.354,P﹤0.05)。[结论]IL-18参与了2型糖尿病肾病患者的发病过程,可以作为糖尿病肾病诊断和肾脏损害程度的临床实验室观察指标。

THE CHANGE OF SERUM IL-18 IN PATIENTS WITH DIABETIC NEPHROPATHY

[Objective]To investigate the change of serum IL-18 in patients with diabetic nephropathy.[Methods]67 patients with diabetic mellitus were divided into two groups:diabetic mellitus without nephropathy group(DM,n = 37)and diabetic nephropathy group(DN,n = 30).30 healthy subjects were chosen to control group.Serum fasting blood glucose(FPG),glycosylated hemoglobin(HbA1c),quantitative level of 24-hour urinary albumin(24h-UA)and IL-18 were measured in all subjects.[Results]Compared to those in the control group,level of FPG,HbA1c and IL-18 in the DM group and the DN group were significant higher.Although FPG and HbA1c were not significantly different between the DM group and the DN group,24h-UA and IL-18 in the DM group were higher than those in the DN group.IL-18 had a good positive correlation(r = 0.354,P﹤0.05)to 24h-UA in the diabetic nephropathy patients.[Conclusion]IL-18 may take part in the pathogenesis of diabetic nephropathy.It can be used to diagnose diabetic nephropathy and judge the severity of kidney damage.
