85例慢性充血性心力衰竭患者血清B型利钠肽与白蛋白的关系分析

目的 探讨充血性心力衰竭（CHF）患者的血清B型利钠肽（BNP）与低白蛋白血症之间的关系.方法 回顾性收集、分析了85例充血性心力衰竭患者临床资料,患者左室射血分数（LVEF）低于50％,无缺血性心脏病,平均随访时间38个月.通过统计分析,探讨BNP或白蛋白浓度基线和基本临床特征,明确BNP与白蛋白浓度变化的相关因素以及每个指标的临床意义.结果 BNP对数转换值与心功能分级、总蛋白水平和左室射血分数相关,而白蛋白与糖尿病史和总蛋白水平呈正相关.BNP和白蛋白浓度的基线值与随访值之差相关（P＜0.0001）.BNP和白蛋白浓度是随访心脏不良事件的独立预测指标.结论 BNP和白蛋白与不同的临床特征相关,其长期变化呈负相关,两者都是独立的预测充血性心力衰竭的指标.

Relationship Between the BNP and 85 Patients with Chronic Congestine Heart Failure

Objective To explore the relationship between B-type natriuretic peptide （BNP） and hypoalbuminemia of patients with congestive heart failure （CHF）.Methods 85 patients with congestive heart failure were collected and analyzed retrospectively. The patients with left ventricular ejection fraction （LVEF） 〈 50% ,no evidence of ischemic heart disease,and an average 38 months follow up. To explore baseline and basic clinical features of the BNP or albumin concentration by statistical analysis, and clear the relevant factors and clinical significance of each indicator between BNP and concentration changes of albumin. Results BNP by digital conver- sion related to LVEF, cardiac function and total protein, albumin were positively correlated with a history of diabetes and total protein. Baseline of BNP and albumin concentration related to the the difference of the follow- up value （P 〈 0. 000 1 ）. BNP and al- bumin concentration are independent predictors of adverse cardiac events in follow up. Conclusion BNP and albumin were associated with different clinical features,and its long- term changes in a negative correlation,both are independent predictors of CHF indicators.
