BMI与老年严重膝骨关节炎发病的相关性研究

摘    要:目的 探讨不同BMI分级、膝关节KSS评分与膝骨关节炎(OA)发病的相关性.方法 选择2011年1月1日~2012年12月31日,年龄在50岁以上行全膝关节表面置换术的膝OA患者214例,根据患者身高、体重计算出BMI.按照我国成年人体重指数标准划分为体重过轻、体重正常、超重、肥胖四组,分别记录各组患者术前膝关节KSS评分,采用SPSS 17.0统计软件进行分析,探讨BMI与膝OA发病的关联性.结果 (1)本组患者总人数214人,其中男88例,女126例,性别比例男:女=0.70;年龄50~85岁,平均68岁,其中,60~69岁组和70~79岁组最多,分别为90例和85例,各占总人数的42.06%和39.72%;(2)本组老年严重膝OA患者BMI与膝关节KSS评分呈低度线性相关,有统计学意义(r=0.135,P〈0.05),两变量存在一定程度的线性相关;各BMI分组中年龄无明显差异;(3)本组老年严重膝OA患者中女性比例高于男性,女性BMI超重人数比例大于男性,有统计学意义(2=7.91,P〈0.05);(4)本组老年严重膝OA患者中,随着BMI的增加,患病人数有增加的趋势,呈正相关,相关系数有统计学意义(r=0.945,P〈0.05).本组患者中以Ⅲ级及Ⅳ级为主;(5)本组老年严重膝OA患者中,随着膝关节KSS评分的降低,患病人数有增加的趋势,呈负相关,相关系数(r=-0.944,P〈0.05),有统计学意义.本组患者中以膝关节KSS评分差(<60分)为主.结论 体重指数与膝OA呈低度线性相关,两变量存在一定程度的线性相关,说明体重指数是老年严重膝OA发病的一个危险因素,但并非其严重化的主要因素;膝OA发病中男女存在差异,女性比男性更易患本病;肥胖程度对膝OA发病的影响较大,两者存在一定的关联性,是膝OA发生发展的一个重要因素.

Abstract:Objective A retrospective study of over-50s old serious knee osteoarth- riffs patients of the pathogenic factors BMI. Combined with BMI classification and KSS knee functional score, To discuss the correlation of knee osteoarthritis pathogenesis and different BMI classifications as well as KSS knee functional score and. Methods From January 1, 2011 to December 31, 2012, 214 patients with knee osteoarthritis and over 50 years old mlderwent TKA . The BMI was calculated by the height and weight. According to China's adult body mass index standard, all the patients were divided into four groups including underweight, normal weight, overweight and obesity. KSS knee functional score of each patient was recorded respectively, and tile results were analyzed by SPSS 17.0 statistical software to discuss the correlation of BMI and knee osteoarthritis pathogenesis. Results (1) There were 88 males and 126 females, and the male/female ratio was 0. 70. The age was between 50 and 85 years old, with an average of 68 years old. There were 90 cases in the 60 - 69 years old group and 85 cases in 70 - 79 years old group, which were 42.06% and 39.72% of the total. ( 2 ) The BMI and knee KSS scores were low linear correlation in the severe osteoarthritis patients ( r = 0. 135 ) with statistical significance ( P 〈 0. 05 ) ; there was no significant differences in age between each BMI group. (3) The proportion of the female was higher than the male in severe knee osteoarthritis, and the number of the overweight female was larger than the male according to BMI ratio (X^2 = 7.91, P 〈 0. 05 ), which was statistically significant. (4) The number of the severe knee osteoarthritis patients had a trend of increase along with the increase of BMI, which was positively correlated ( correlation coefficient r = 0. 945, P 〈 0. 05 ). ( 5 ) The number of the severe knee osteoarthritis patients had a trend of increase along with the knee KSS score decrease, which was a negative correlation (the correlation coefficient r = -0. 944, P 〈 0. 05 ). Conclusions The BMI and the knee osteoarthritis are low linear correlation. The certain degree of linear correlation of the two variables means that BMI is a risk factor of the severe knee osteoarthritis pathogenesis in the elderly, but not the main factor. There are differences between the male and the female in the knee osteoarthritis pathogenesis, and the female patients are more likely to suffer from the disease than the male patients. Obesity has a powerful influence in the knee osteoarthritis pathogenesis which is an important factor of the knee osteoarthritis occurrence and development.
