前置胎盘合并胎盘植入20例临床分析

目的探讨前置胎盘合并胎盘植入的高危因素及对妊娠的影响与诊断处理方法。方法我院产科住院分娩的221例前置胎盘孕产妇,其中合并胎盘植入20例作为实验组,其余201例作为对照组,对其临床资料进行分析。结果实验组孕产妇年龄、孕次≥3次、流产2次以上、瘢痕子宫及中央型前置胎盘的发生率均高于对照组,两组比较差异有统计学意义(P<0.01);实验组产后出血及休克的发生率均明显高于对照组,差异有统计学意义(P<0.01)。结论孕妇年龄、孕次、流产、瘢痕子宫以及中央型前置胎盘均是前置胎盘合并胎盘植入的高危因素,前置胎盘合并胎盘植入可引起产时产后出血量增多,重视早期诊断,及时处理,从而降低母婴并发症的发病率。

The Clinical Analysis of 20 Cases Placenta Previa combined with Placenta Accrete

Objective To explore the high risk factors and pregnancy influence and diagnosis and management of placenta previa combined with placenta accrete.Methods The complete clinical data of 221 patients of placenta previa were analyzed,20 cases of which combined with placenta accrete as the experimental group and the others as the control group,the clinical data were analyzed.Results The age,pregnancy over three times,abortion over two times,scar uterine and the rate of central placenta previa of the experimental group were higher than the control group,there were statistically difference（P 0.01）;The amount of postpartum hemorrhage and the incidence of shock of the placenta previa combined with placenta accrete group were higher than the group of placenta previa,there were statistically difference（P0.01）.Conclusion Age,pregnancies,abortion,scar uterine and the rate of central placenta previa were the high risk factors of placenta previa combined with placenta accrete,it can cause hemorrhage increased,We should pay attention to the early diagnosis and timely treatment in order to reduce the incidence of the complications of mother and child.
