2009～2013年大连市甘井子区46例重症高危孕产妇管理分析

目的通过对2009～2013年甘井子区46例重症高危孕产妇管理情况分析，找出重症高危孕产妇管理的薄弱环节，提高围生期保健管理质量，降低孕产妇死亡的发生。方法收集2009～2013年甘井子区46例重症高危孕产妇资料。对重症的发生、报告、会诊、转运、救治及结局进行分析总结。结果46例重症孕产妇中高龄产妇14例，足月妊娠27例，前三位疾病因素为：子痫前期（重度）、胎盘早剥、产后出血。46例产妇均抢救成功，死亡0例。结论加强流动人口孕产妇系统管理，全面建立基层高危妊娠的专案管理，提高助产机构对重症高危孕产妇早期识别与处理的能力．严格执行重症管理绿色通道制度，是确保母婴安全、降低胎婴儿死亡率和病残率的重要环节。

Management analysis of 46 cases with critically ill high risk pregnant women and puerperae of Ganjingzi district in Dalian city from 2009 to 2013

Objective To analyze the management of 46 cases with critically ill high risk pregnant women and puerperae in Ganjingzi District from 2009 to 2013,find out the weak point on managing severely high risk pregnant and lying-in women,in order to improve management quality of health care during perinatal period,and decrease the occurrence of death in pregnant and lying-in women. Methods Data from 46 cases with critically ill high risk pregnant women and puerperae in Ganjingzi District from 2009 to 2013 were collected,occurrence,reporting,group consultation,transferring, rescuing,and outcomes were analyzed and summarized. Results Among all these critically ill pregnant and lying-in women,the number of elderly parturient woman was 14 and full-term pregnancy was 27.The top 3 disease factors were preeclampsia （critical condition）,abruptio placentae,and postpartum hemorrhage.All these critically ill pregnant and lying-in women were successfully rescued,and nobody was dead. Conclusion Important elements including strengthening system management of pregnant women and puerperae in floating population,comprehensively establishing primary-level project management on high risk pregnancy,improving early identification and disposal in critically ill high risk pregnant and lying-in women in midwifery institution,and strictly carrying out green channel system of critically ill management can ensure safety of mother and infant and reduce the mortality and disability rates in fetus and infant.
