高频彩色多普勒超声加水囊对肢体浅表组织病变的诊断价值

目的：探讨高频彩色多普勒超声加水囊（WC）对肢体浅表组织表面凹凸不平病变的诊断价值。方法对229例各种肢体浅表组织表面凹凸不平的病变采用高频彩色多普勒超声直接检查和高频彩色多普勒超声加WC间接检查，全部病例均以手术或穿刺病检作为金标准用以确诊，在3种方法之间比较其诊断准确率。结果与手术或穿刺病检比较，高频彩色多普勒超声加WC对脂肪瘤、纤维瘤、表皮样囊肿、腱鞘囊肿、跟腱断裂、手指肌腱断裂诊断准确率分别为98.5%、94.7%、100%、100%、100%、95.2%，对肢体浅表组织表面凹凸不平病变的诊断总准确率为98.7%，总误诊率为1.3%；用高频彩色多普勒超声直接检查的诊断总准确率为87.2%，总漏诊率为12.8%。结论利用高频彩色多普勒超声加WC对肢体浅表组织表面凹凸不平的病变进行超声探查，可使病变的结构层次更加清晰并明显改善图像质量，大大提高超声诊断准确率。

Diagnosis values of high frequency color Doppler ultrasound with water cyst for pathological changes of limbs'superficial tissues

Objective To explore the diagnosis values of the high frequency color Doppler ultrasound with water cyst （WC） for the pathological changes of limbs’superficial tissues. Methods The positions of pathological changes for 229 cases with the limbs’irregular superficial tissues were searched in contrast with applying that direct detection was made by the high frequency color Doppler ultrasound,indirect detection was conducted by high frequency color Doppler ultrasound with WC.The diagnosis of all patients used the operation,puncture and pathology as the gold standard.The diagnostic accurate rates of three methods were compared. Results The diagnostic accurate rate of the high frequency color Doppler ultrasound with WC for the lipoma,fibroma,epidermoid cyst,thecal cyst,rupture of the achilles tendon and finger tendon rupture was 98.5%,94.7%,100%,100%,100% and 95.2% respectively,compared with the operation,puncture and pathology,the total diagnostic accurate rate of this method for the pathological changes of irregular surface in the limbs’superficial tissues was 98.7%and total misdiagnosis rate was 1.3%.However,the total diagnostic accurate rate of the high frequency color Doppler ultrasound was 87.2% and total missed diagnosis rate was 12.8%. Conclusion The high frequency color Doppler ultrasound with WC used to ultrasound expedition for the pathological changes of irregular surface in the limbs’superficial tissues is usable more clear for the layer of structure of patho logical changes,obviously improve the quality of image and can greatly heighten the accurate rate of ultrasound diagnosis.
