双螺旋CT三维重建在探测鼻骨外伤性骨折的应用

摘    要:目的探讨簿层双螺旋CT检查对鼻部外伤的诊断价值。方法对48例鼻部外伤患者行螺旋CT检查,层厚1mm,螺距1:1,骨算法靶扫描,三维重建。结果 48例患者均经CT扫描显示了鼻骨骨折,其中,伴发上颌骨额突骨折32例,鼻颌缝分离36例,鼻间隙分离14例,鼻中隔骨折16例,眼眶骨折16例。结论螺旋CT3D重建能清楚显示鼻部及相邻器官解剖结构的空间关系,对鼻部骨折的部位、移位情况及并发改变能细致、全面、准确地显示,可鉴别鼻骨滋养孔与骨折线,因而它是鼻部外伤性骨折准确而简便的影像学检查方法,尤其能为术前治疗方案的设计提供有价值的信息。

Abstract:Objective To evaluate the usefulness of double spiral CT with three dimensional(3D) reconstruction in the identification of nasal bone fractures.Methods Forty-eight patients with clinically suspected nasal bone fractures underwent CT scaning and 3D reconstruction with thickness 1.0 mm,pitch 1:1,and bone algorithm.Results CT scanning and 3D reconstruction showed the nasal bone fractures in all the 48 cases,of whom,coexisted the fractures of maxillary-frontal process were found in 32,the separation of nasofrontal suture in 36,the separation of internasal space in 14,the fractures of nasal septum in 16,and the fractures of orbital walls in 16.Conclusion 3D-CT reconstruction is the optimal imaging technique for the diagnosis of nasal bone fracture and its coexisted other fractures and can provide valuable information of space relationship,especially for the desigh of treatment plan before operation.
