外伤性脾破裂患者脾切除术后远期免疫功能观察

目的评价外伤性脾破裂行脾切除对患者远期免疫功能的影响.方法随访昆明市延安医院2004年8月至2006年8月2a间因外伤性脾破裂行脾脏切除的患者87例,测定其免疫功能,与正常对照组（21例）进行比较.结果18例失访,其余69例随访时间3～5a,术后免疫球蛋白IGG、IGA较对照组升高,IGM与对照组相比无显著差异;补体C3、C4无明显变化;淋巴细胞水平,CD3细胞亚群、CD4及CD8细胞亚群均较对照组升高;CD4/CD8比值两组相比无明显差异.结论对于成人,外伤性脾破裂行脾脏切除不会明显降低患者的远期免疫功能.

Analysis of the Immune Function in Patients with Traumatic Splenic Rupture After Splenectomy

Objective To investigate the effects of splenectomy on the immune function of patients with traumatic splenic rupture.Methods 69 cases with traumatic splenic rupture after splenectomy were followed up,and the laboratory index of immune function were tested and compared with control group.Results We followed up for 3 to 5 years.The plasma concentration of IGG and IGA postsplenectomy were higher than control group,the change of IGM,C3 and C4 were not significant.The lymphocyte counts,the subsets of CD3,CD4 and CD8 postsplenectomy were higher than control group,The change of CD4/CD8 ratio was not significant. Conclusion Splenectomy does not decrease the immune function for adults, splenectomy shuld be considered firstly for traumatic splenic rupture.
