雾化吸入糖皮质激素对外科术后合并呼吸机相关性气管支气管炎患者的影响

【摘要】 目的 探讨外科术后发生呼吸机相关性气管支气管炎(VAT)患者使用雾化吸入糖皮质激素对VAP发生率及其预后的影响。方法 将2012年1月~2013年4月入住我院的92例术后VAT患者随机分配到糖皮质激素雾化组和对照组,治疗期7天,观察期3个月,比较两组患者呼吸机相关性肺炎(VAP)发生率、有创机械通气时间、入住ICU时间、死亡率情况。结果 治疗组的VAP发生率及死亡率显著低于对照组;在ICU住院日、机械通气时间等方面比较差异均无统计学意义(P>0.05)。结论 对于发生VAT的外科术后患者,雾化吸入糖皮质激素可能降低术后患者VAP的发生率。

Effect of aerosol inhalation of glucocorticoid on the postoperative patients with ventilator-associated tracheobronchitis

Objective To evaluate the effect of glucocorticoid nebulization to the postoperative patients with ventilator-associated tracheobronehitis （VAT）. Methods Ninety-two patients postoperative VAT were observed in Guangzhou First People＇s Hospital. The patients were divided into 2 groups randomly：glucocorticoid group and control group, treatment for 7 days, observed peraid for 3 months. The incidence of, mortality rate,days of mechanical ventilation and average ICU time were comparel between two groups. Results The incidence of VAP and mortality of treatment group was significantly lower than that control group. But the days of mechanical ventilation and average ICU stay time were no statistically significant （P〉0.05）. Conclusion Nebulized glucocorticoids can reduce the incidence of VAP for postoperative patients with VAT.
