36例原发性胆囊癌的诊断及外科治疗分析

目的探讨胆囊癌的早期诊断方法及外科治疗手段。方法对1988～2008年收治的、经手术及病理证实的36例胆囊癌患者进行术前检查、术中发现及病理结果分析。结果本组胆囊癌术前早期确诊率低。术中发现中晚期病例居多。手术治疗效果差。病理组织学分类以腺癌(24/36)、鳞癌(6/36)为主。结论早期诊断、早期手术是提高胆囊癌治愈率的关键。为预防胆囊癌的发生,对胆囊结石、胆囊息肉中可疑的高危患者可进行早期手术治疗。

Analysis on early diagnosis and surgical treatment of primary gallbladder carcinoma in 36 cases

Objective To investigate the early diagnosis and surgical treatment for carcinoma of gallbladder.Methods 36 cases of carcinoma of gallbladder confirmed by operation and pathological examination were analyzed retrospectively in our hospital from 1988 to 2008.Results The early diagnosis of carcinoma of gallbladder was low proportion in this group,the effects of operation treatment were poor due to more advanced patients of carcinoma of gallbladder discovered in operation.According to the classification of pathology,adenocarcinoma and squamous carcinoma took a large proportion （19/36 and 4/36respectively）.Conclusion It is the key step to increase the rate of recovery of carcinoma of gallbladder only for the early diagnosis and early operation treatment.Meanwhile,it is also necessary to perform early operation treatment（cholecystectomy）to prevent the carcinoma of gallbladder for the suspected patients with high risk factors in cholecystolithiasis and gallbadder polyplesion.
