实行夜班加强辅助班与护理住院总值班对妇产科夜班护士压力的影响

目的 探讨通过实行夜班加强辅助班及护理二线住院总查房制度,对减轻妇产科夜班护士压力的影响.方法 通过对夜班实行加强辅助班及开展夜间护理住院总值班制度,实施前后采用护士工作压力量表对48名一线夜班护士进行的压力状态比较.结果 实施后护士在患者护理方面、工作量及时间分配方面、护理专业及工作方面的压力得分分别为（1.91±0.32）,（1.73±0.48）,（1.48±0.61）分,均低于实施前的（3.35±0.76）,（3.35±0.47）,（2.92±0.67）分,差异具有统计学意义（t值分别为2.44,2.35,2.25;P ＜0.05）,管理及人际关系方面的问题、工作环境及资源方面实施前后比较,差异无统计学意义（P＞0.05）；在实施加强班和护理住院总值班后,夜班护士前5位压力源均减轻,与实施前比较差异均有统计学意义（P＜0.01）.结论 通过实行夜班加强辅助班和护理总值班制度,减轻了妇产科一线夜班护士的压力,保障了护理质量安全.

Effect of the implementation of the reinforced night shift and the general resident nurse on duty system on relieving the stress of night nurse in the department of obstetrics and gynaecology

Objective To investigate and discuss the influence of the implementation of the reinforced night shift and the general resident nurse on duty system on relieving the stress of night nurse in the department of obstetrics and gynaecology.Methods A total of 48 first-line nurses on duty at night were selected and their stress condition were investigated with Nurse＇ s Work Pressure Sources Scale and compared between before and after the implementation of the reinforced night shift and the general resident nurse on duty system.Results After the implementation,the stress scores of nurse in nursing care for patients,in the workload and time allocation and in the nursing profession and nursing work weres lower than those before the implementation [（1.91 ±0.32） vs （3.35 ±0.76）,（1.73 ±0.48） vs （3.35 ±0.47）,（1.48 ±0.61） vs （2.92 ±0.67）;respectively],these differences were statistically significant （t=2.44,2.35,2.25,respectively;P ＜ 0.05）.There were no statistically significant differences regarding the problems of management and interpersonal relationship and the problems of work environment and resources between before and after implementation （P ＞0.05）.The stressors of night shift nurses in the top 5 were all reduced after the implementation of the reinforced night shift and the general resident nurse on duty system,and the differences were statistically significant between after and before the implementation （P ＜ 0.01）.Conclusions The implementation of the reinforced night shift and the general resident nurse on duty system can relieve the stress of first-line nurses on duty at night in the department of obstetrics and gynaecology,which can ensure the quality and safety of nursing.
