Survivin蛋白预测直肠癌术前放疗敏感性的临床意义

目的探讨Survivin蛋白表达与直肠腺癌术前放疗病理反应程度的关系，评价Survivin蛋白表达水平在预测直肠癌术前放疗敏感性的价值。方法63例采用术前放疗的直肠腺癌患者，评价其放疗后病理反应程度，免疫组化方法检测手术治疗前活检标本和手术后切除标本中Survivin蛋白的表达水平，分析活检组织中Survivin蛋白表达水平与病理反应程度的关系，以及放疗前后蛋白表达水平的变化。结果直肠癌术前放疗后病理反应良好，有效率达71％，联合放化疗者优于单纯放疗者（P〈0．05）。单纯放疗组Survivin蛋白表达高低与放疗疗效相关（P〈0．05），同期放化疗组则显著相关（P〈0．01）。两组均显示Survivin蛋白表达高的，放疗效果差。肿瘤组织放疗后Survivin蛋白表达水平无明显下降。结论直肠癌组织中Survivin蛋白表达水平可作为术前放疗敏感性的预测因子。

Survivin as predictive marker of tumor response to preoperative radiotherapy in rectal cancer

Objective To investigate the correlation of Survivin immunoreaetivity in tumor tissues to tumor response to preoperative radiotherapy in rectal cancer, and to value Survivin as a predictive marker of tumor response. Methods Clinical data of 63 patients with rectal cancer undergoing preoperative radiotherapy were reviewed. Pathologic tumor response to irradiation was evaluated. The expression of Survivin in tumor tissues before irradiation and after operation was detected by immunohistoehemistry. The correlation of Survivin irnmunoreactivity in tumor biopsies before irradiation to pathologic tumor response to irradiation was analyzed. Results Preoperative radiation in rectal cancer resulted in a considerable tumor response, with a response rate of 71%. Tumor responses were better in the patients who received preoperative radiochemotherapy than in those received preoperative radiotherapy alone （ P 〈 0. 05 ）. The positive expression of survivin before radiotherapy higher, the total efl＇ieaey better, there was statistically significant correlated in those received preoperative radiotherapy alone （ P 〈 0. 05）, and in those received preoperative radiochemotherapy （ P 〈 O. O1 ）. The expression of Survivin in tumor tissues was not statistically descended after preoperative radiation. Conclusions Survivin may be a marker of tumor response to Dreooerative radiotherunv in metal cancer.
