survivin、p27和cyclin D1在胃腺癌组织中的表达及其临床意义

目的：探讨survivin、p27和cyclinD1在胃腺癌组织中的表达及其与肿瘤临床病理特征之间的关系。方法：用免疫组织化学S-P法检测73例胃腺癌及20例正常胃黏膜组织中survivin、p27和cyclinD1的表达情况。结果：①胃腺癌组织中survivin和cyclin D1蛋白表达阳性率分别为46.6%（34/73）和60.3%（44/73）,明显高于它们在正常胃黏膜组织中的表达率0%（0/20）和25%（5/20）（P〈0.01,P〈0.01）;p27在胃腺癌组织中蛋白表达阳性率为41.1%（30/73）,明显低于它在正常胃黏膜组织中的阳性表达率95.0%（19/20）（P〈0.01）。②survivin、p27和cyclinD1均与胃腺癌组织分化程度无明显相关性（均P〉0.05）;survivin与胃腺癌浸润深度、淋巴结转移及临床分期呈正相关（均P〈0.05）;p27与胃腺癌浸润深度、淋巴结转移及临床分期呈负相关（均P〈0.01）;cyclinD1仅与胃腺癌的浸润深度相关（P〈0.05）。③胃腺癌中survivin的阳性表达率与p27呈负相关（P〈0.05）,而与cyclinD1的阳性表达率呈正相关（P〈0.01）;p27的阳性表达率与cyclinD1无明显相关性（P〉0.05）。结论：survivin、p27和cyclinD1基因在胃腺癌的发生、发展中起着不同程度的作用;联合检测survivin、p27和cyclinD1可能对胃腺癌恶性程度的判定及胃腺癌的预后评估提供帮助,并对进一步治疗提供一定的依据。

Expression of survivin, p27 and cyclin D1 in gastric adenocarcinoma and its clinical significance

Objective： To investigate the protein expression of survivin, p27 and cyclinD1 in gastric adenocarcinoma and study the relationship between their expression and clinicolpathological features. Methods： Immunohistochemical method was used to analyze protein expression of survivin, p27 and cyclin D1 in paraffin embedded tissues from 73 cases of gastric adenocarcinoma and 20 cases of normal gastric mucosa tissues. Results：①The positive rate of survivin and cyclinD1 protein expression was 46.6%（34/73）and 60.3%（44/73）in gastric adenocarcinoma, higher than those in normal gastric mucosa tissues0%（0/20） and 25%（5/20）（P〈0.01,P〈0.01）, The positive rate of p27 protein expression was 41.1% （30/73）in gastric adenocarcinoma, lower than that in normal gastric mucosa tissues 95.0%（19/20） （P 〈0.01）, ②The expression of survivin, p27 and cyclinD1 all incorrelated with differentiated depth of gastric adenocarcinoma, The expression of survivin positively correlated with infiltrative depth, lymph node metastasis and clinicolpathological stage（P〈0.01,P〈0.01,P〈0.01）, The expression of p27 negatively correlated with infiltrative depth, lymph node metastasis and clinicopathologic stage（P〈0.01,P〈0.01,P 〈0.01）, and the expression level of cyclin D1 only correlated with infiltrative deepness of gastricadenocareinoma （P〈0.05）. ③Significant relationships were found between survivin and p27 or cyclin D1 （P〈0.05,P〈0.01）, while no correlation was found between p27 and cyclin D1（P〉0.05）. Conclusion：The experiment suggest that survivin, p27 and cyclin D1 may play roles in the oncogenesis and progression of gastric adenocarcinoma, but to different extents, respectively. Combinated detection of Survivin, p27 and cyclin D1 may be helpful to judge malignancy and evaluate prognosis and treatment of gastric adenocarcinoma.
