摘    要:目的了解重症肺炎患者部分血栓前状态(PTS)相关指标变化,探讨其临床意义。方法回顾性分析88例重症肺炎和80例健康体检者血浆凝血酶原时间(PT)、活化部分凝血活酶时间(APTT)、纤维蛋白原(Fg)、D-二聚体(D-D)和纤维蛋白降解产物(FDP)水平,并计数血小板(PLT)和白细胞(WBC)。结果重症肺炎组PLT降低,WBC增高,PT、APTT延长,Fg增高,D-D及FDP阳性,与对照组比较,差异有统计学意义(P〈0.05)。重症肺炎组PLT水平与PT、APTT、Fg呈显著相关(r=0.416、0.521、0.458)。重症肺炎组WBC水平与PT、APTT、Fg呈显著相关(r=0.387、0.524、0.486)。结论重症肺炎患者存在凝血、纤溶系统功能的紊乱,可能与炎性反应有关。注意PTS相关指标的监测,对于重症肺炎的诊治具有重要意义。

Abstract:Objective To observe part of the indexes about pre-thrombotic state(PTS) in patients with severe pneumonia,and to explore the clinical meaning. Methods To analyse the index of PT, APTT, Fg, D-Dimer and FDP, and count the number of both the planet(PLT) and white blood cells(WBC) of 88 patients with severe pneumonia and 80 healthy persons retrospectively. Results Compared with the control group, the number of PLT of the group with severe disease decreases while WBC increases,PT and APTT prolongs,and the value of Fg is raised,D-D and FDP become positive in the group with severe pneumonia(P〈0.05). Level of PLT is related to PT, APTT and Fg(r=0. 416,0. 521,0. 458),and so is WBC(r=0. 387,0. 524,0. 486). Conclusion The mechanism of coagulation and fibrinolytic function becomes disorder for the patients with severe pneumonia. It may be related to the inflammation. Important attention should be paid ed to the index,which might be helpful for the diagnosis and treatment of severe pneumonia.
