慢性呼吸衰竭患者外周血白细胞糖皮质激素受体改变的研究

应用放射配体结合法和放射免疫法分别检测了慢性支气管炎伴及不伴慢性呼吸衰竭的患者外周血白细胞糖皮质激素受体（ＧＲ）和血浆皮质醇（Ｆ）浓度。结果：两实验组患者的ＧＲ最大结合容量（Ｒ＿０）均较健康对照组明显降低（Ｐ均＜０．０１），呼吸衰竭组又较无呼吸衰竭组低（Ｐ均＜０．０５）；而ＧＲ的亲和力（平衡解离常数，Ｋｄ）和血浆Ｆ浓度３组间无显著差异（Ｐ＞０．０５）。结果表明：慢性呼吸衰竭患者外周血白细胞ＧＲ的改变为继发性，其在疾病的发生和发展过程中可能具有代偿意义，也产生不良影啊。

Changes of glucocorticoid receptor in peFipheral leuko-cytes in patients with chronic respiratory failure

The maximum binding capacity(R0)and dissocia-tion constant(Kd) of glucocorticoid receptor(GR)inperipheral leukocytes were estimated by radioligandbinding and radioimmunologic methods and plasma corti-sol(F) level measured in the patients with chronic bron-chitis with and without chronic respiratory failure(n =12 and n=9 respectively). The R_0 of GR in the two ex-periment groups was significantly lower than that of thehealthy controls(P< 0.01),also,it was significantlylower in the patients with respiratory failure than inthose without respiratory failure(P< 0.05 ). No signifi-cant differece was observed in the Kd and the F betweenthe two experiment groups and the healthy controls(P>O.05 ),This study showed that the changes of GR in theperipheral leukocytes in the patients with chronic respi-ratory failure were secondary,and might have compen-satory significance,but simultaneously, might preduceharTnhl effects.
