重型颅脑损伤患者早期肠内营养支持对预后的相关性研究

摘    要:目的探讨重型颅脑损伤患者早期实施肠内营养支持与预后的相关性。方法对64例重型颅脑损伤患者(格拉斯哥昏迷评分≤8分),采用十二指肠置管早期实施肠内营养支持作为治疗,64例重型颅脑损伤患者(格拉斯哥昏迷评分≤8分)采用传统的延迟性胃肠内营养的同类患者作为对照组,进行有关的营养指标、并发症、预后的临床观察及统计学处理,并进行比较。结果重型颅脑损伤患者能从早期的肠内营养途径中获得比较充分的能量和蛋白质合成所必需的成分,治疗组营养指标均优于对照组(P〈0.05),治疗组早、晚期并发症均少于对照组,预后优于对照组(P〈0.05)。结论对重型颅脑损伤患者采用十二指肠置管进行早期肠内营养支持可减少并发症,改善预后。

Abstract:Objective To find the relativity about early enternal nutrition and prognosis in severe craniocerebral injury. Methods 64 patients with severe eraniocerebral injury (GCS≤8分) were given early Enternal Nutrition (EN) by duodenal catheter as observed group, 64 patienes with severe cranioeerebral injury (GCS≤8 分) were given conventional delayed EN as eontral group. Nutritional parameters, complications and prognosis were observed and compared in observed group and control. Results With early EN by duodenal catheter, the patients could receive adequate energy and ingrodienes of protein synthesis, the nutritional parameters of observed group were superior to the controls ( all P 〈 0.05 ), the early or late complications were less than the controls, and the prognosis were marked improved compared with the controls ( all P 〈0.05 ). Conclusion Early EN in the severe craniocerebral injury patients by duodenal catheter could decrease complications and comprere prognosis of the batients.
