骨质疏松腰椎椎体形态变化规律的影像学研究

摘    要:目的:通过对骨质疏松腰椎椎体的影像学形态变化的观察,探讨形态变化规律.方法:选取骨质疏松患者35例,骨密度正常患者35例,分别拍摄腰椎X线片(L1~L4),测量椎体前、中、后高度及平均矢状径.计算椎体前、中高与后高的比值及变形指数.结果:与正常组相比,骨质疏松组椎体高度明显下降(P<0.05).前高/后高两组差异显著(P<0.05);中高/后高L1,L2差异不显著,L3,L4差异显著(P<0.05).结论:腰椎骨质疏松时椎体生物力学强度下降,导致椎体压缩变形,最常见为楔形变.

Abstract:Objective: To investigate changing rules of the morphology of lumbar vertebral bodies in osteoporosis by observing the X-ray changes in lumbar vertebral bodies.Methods:X-ray films of lumbar vertebrae(L_1 to L_4) were taken in 35 patients with osteoporosis and 35 normal subjects,respectively.The anterior,middle,and posterior height(ha,hm,and hp) and the average sagittal diameter of vertebral bodies were measured,and the height ratio and deformation degree were calculated.Results:The height of vertebral bodies in patients with osteoporosis decreased significantly than that in normal subjects(P<0.05).There were significant differences in anterior height/posterior height in L_1 to L_4 lumbar vertebrae.The difference of middle height/posterior height was significant between L_3 and L_4(P<0.05),but not significant between L_1 and L_2(P>0.05).Conclusion:The biomechanical strength of vertebral bodies decreases in osteoporosis,which results in compressed deformation.Cuniform deformation is the most common form.
