非典型鳞状细胞意义不明中的宫颈高度病变

目的探讨非典型鳞状细胞意义不明(atypical squamous cells of undetermined significance,ASC-SU)对诊断宫颈上皮内瘤变(cervical intraepithelial neoplasia,CIN)2级及其以上病变的临床意义。方法对2009年在北京大学第三医院进行宫颈涂片检查的妇女的资料进行回顾性分析。结果 2 163例宫颈细胞学为ASC-US,546例行阴道镜检查及宫颈活检病理检查,57例病理结果为CIN2级及其以上病变,复核细胞学符合率约50%。57例中55例在阴道镜下可见异常图像。56例行高危型人乳头瘤病毒(high-risk human papillomavirus,HR-HPV)检测,55例检测结果为阳性。结论联合应用HR-HPV检测、阴道镜检查和宫颈活检术可提高ASC-US妇女的宫颈高度病变检出率。

High-Grade Cervical Intraepithelial Neoplasia and Cervical Cancer in Atypical Squamous Cells Undetermined Significance

Objective To detect the role of atypical squamous cells undetermined significance（ASC-US） in cervical intraepithelial neoplasia（CIN） 2,CIN 3 and cervical cancer（CIN 2＋）.Methods The women with ASC-US cytologic test and performed cervical colposcopy-directed biopsy pathology test were included in this retrospected study from January 1st 2009 to December 31st 2009.Results Among 2163 cases,546 were referraled to colposcopist,and 57 cases were diagnosed CIN 2＋.The cytology slides of the 57 cases were reviewed,and 28 diagnosis should be modified.The colposcopy images were reviewed,abnormal images were detected in 55 cases.High risk human papillomavirus（HR-HPV） tests were performed in 56 cases,positive in 55 cases.Conclusion Our data suggests that it is helpful in detection CIN2/3＋ from ASC-US women to detect HRHPV and combine applications of colposcopy and biopsy.
