围手术期输血对结肠癌患者预后的影响

目的:通过大宗结肠癌病例的临床病理特征分析,探讨围手术期输血对结肠癌患者术后生存的影响。方法:对10年内723例结肠癌手术治疗病例预后进行了回顾性单因素分析。结果:围手术期输血≥800mL可显著降低患者生存率(P0.05),特别对40岁以下行右半结肠切除术、乳头状腺癌、TNMⅠ期、一站淋巴结转移(N1+)、以及无肝转移患者的预后有显著性不良影响(P0.05)。结论:围手术期输血对上述结肠癌患者的预后有一定不良影响,因此对此类患者应严格掌握输血适应证,但对Ⅳ期病例及有远处转移者的预后围手术期输血并无不良影响,输血的适应证对此类病人应当放宽。

Influence of Perioperative Blood Transfusion on Prognosis in Patients with Colon Cancer

Objective To analyze the influence of blood transfusion on prognosis of patients with colon cancer. Methods Univariate retrospective analysis of survival was done in 723 colon cancer patients which were treated surgically during a period of 10 years. Results Colon cancer patients who received more than 800 mL perioperative blood transfusion had a reduced survival （P 0.05）. Blood transfusion influenced significantly the prognosis of patients 40 years old and younger, patients underwent right hemicolectomy, patient with papillary adenocarcinoma, patients with stage I tumors, patients with lymphatic node metastases and patients without liver metastases （P 0.05）. Conclusion Perioperative blood transfusion is a prognostic factor for patients with colon cancer. The indication of blood transfusion must be restricted strictly. But perioperative blood transfusion may not be deleterious for patients with staging IV diseases and with distance metastases.
