摘    要:目的 :了解慢性阻塞性肺疾病 (COPD)患者气道阻力与呼吸肌功能对中枢驱动的影响。方法 :本实验共包括 77例患者 ,男 4 9例 ,女 2 8例 ,年龄 6 0岁~ 85岁之间 ,分为四组 :①为无阻塞组 ;②COPD所致气道轻度阻塞组 ;③COPD所致气道中度阻塞组 ;④COPD所致气道重度阻塞组。以上无阻塞组为对照组 ,比较采用单因素方差分析 ,组间两两比较采用SNK - q检验。结果 :PIMAX、P0 .1及P0 .1 PIMAX与SReff差别有显著性。PEMAX各组之间差别无显著性。结论 :COPD患者平时处于高中枢呼吸驱动 ,遇感染、心衰等应激情况时将易发生呼吸衰竭 ,而改善呼吸阻力将有助于缓解COPD患者的高中枢呼吸驱动

Abstract:Objective:We study the effect of airway resistance on central drive and respiratory muscle function in patients with COPD. Methods: All 77 patients, aged from 60 to 85 years, were divided into four groups: COPD with slight airway obstruction group, COPD with middle airway obstruction group, and COPD with severe airway obstruction, non-obstruction as the control group. Analysis of variance was used to evaluation, the comparison between each two groups was measured by SNK-q test. Results: There were significant differences between the groups in P 0.1, P 0.1/PIMAX, but not in PEMAX. Conclusion: COPD patients were higher in the levels of central respiratory drive, so that it was more sensitive to infections; The heart failures and other emergencies were easier, and more vulnerable respiratory failures. If the resistance of airway in COPD patients are ameliorated that the higher levels of central respiratory drive can be lyses.
