摘    要:目的 应用实时三维超声心动图评价心脏再同步治疗对左室收缩同步性的影响.方法 15例慢性心力衰竭患者,心功能(NYHA)Ⅲ~Ⅳ级,左室射血分数≤35%,QRS>130 ms.安装双心室再同步起搏器.用实时三维超声心动图技术比较术前术后左室收缩同步性的变化.结果 左室收缩同步性指标Tmsv16-SD%、Tmsv12-SD%由术前(11.52±1.73)、(11.95±1.84)减少到术后3个月的(10.46±1.38)、(10.57±1.53)(P<0.05).结论 实时三维超声心动图作为超声成像领域的一项新技术,在评价心脏机械收缩失同步方面作用日趋重要.

Effect of left ventricular systolic synchronicity in patients with cardiac resynchronization therapy by real-time three dimensional echocardiography

Abstract:Objective To evaluate the effect of left ventricular systolic synchronicity in patients with cardiac resynehronization therapy by real-time three dimensional echocardiography (RT-3 dE). Methods 15 patients fulfilled the standard biventricular pacing therapy indication with New York Heart Association (NYHA) class III- IV, left ventricular ejection fraetion ≤35%, QRS duration 〉 130 ms. All patients were implanted biventricular pacing devices. Three months after implanation of biventricular pacing devices, percent of 16,12 section standard deviation in time to minmal systolic volume (Tmsv 16-SD%, Tmsv12-SD% ) and left ventricu- lar ejection fraction (LVEF). Results Tmsv 16-SD% and Tmsv12-SD% significantly decreased from ( 11.52 ±1.73,11.95 ±1.84) before operation to ( 10.46 ± 1.38,10. 57 ± 1.53 ) 3 months after implanation of biventrieular pacing devices(P 〈0. 05) . Conclusion Real-time three dimensional eehocardiography is a new imaging technique in Echocardiography. It has great importance in evaluating cardiac mechanical dyssynchrony.
