人工晶状体植入治疗先天性白内障56例

摘    要:目的 :观察人工晶体植入手术联合视功能训练对先天性白内障患儿视功能恢复的影响 ,探讨先天性白内障的治疗方法。方法 :对 5 6例 ( 72眼 )先天性白内障人工晶状体植入术后视功能恢复情况进行回顾性分析 ,采用白内障超声乳化术或双腔注吸术 ,掌握手术和人工晶状体植入时机、选择适宜的人工晶状体度数、术中一期后囊膜撕开 ,术后及时进行双眼视功能训练。术后随访 2年以上。结果 :术后 2年 ,矫正视力≥ 0 .3者 5 5眼 ( 76.4% ) ,具有融合功能者 2 4例 ( 4 2 .9% ) ,具有立体视觉者 2 2例 ( 39.3% )。中央视轴区透明者 65眼( 90 .3% ) ,后囊及玻璃体前界膜增殖膜形成者 7眼 ( 9.7% )。结论 :先天性白内障人工晶状体植入手术时机、术式选择恰当 ,术后配合视功能训练可获得较满意的视功能恢复

Abstract:Objective: To evaluate the effect of operation and visual function training on recovery of visual function in children with cataract. Methods: 56 cases (72 eyes) with congenital cataract were treated with extracapsular cataract extraction (ECCE) or Phacoemulsification and intraocular lens (IOL) implantation. The operation time, diopters of IOL, primary posterior capsulotomy and binocular postoperative visual acuity training were analyzed retrospectively. Follow-up lasted for at least 2 year. Results: 2 year after operation, corrected visual acuity of congenital cataract was 0.3 or better in 55 eyes (76.4%), 24 cases (42.9%) had fusion power, 22 cases (39.3%) have stereopsis, 65 eyes(90.3%) remained transparent in the optic area, 7 eyes (9.7%) have proliferative membrane behind the IOL again. Conclusion: Satisfactory visual acuity can be achieved in children with cataract extraction, IOL implantation and visual function training. It is important to choose optimal operation time, methods and postoperative visual acuity training for better recovery of visual function in these children.
