摘    要:目的采用千伏级锥形束断层扫描(kV-CBCT)量化分析胸腹部肿瘤容积旋转调强放射治疗(VMAT)摆位误差的准确度。方法选取2017年3月至2019年9月于福建医科大学附属协和医院放疗科行胸腹部VMAT的78例患者作为研究对象,于治疗前行k V-CBCT获取实时摆位3D扫描验证图像,并采用灰度自动配准结合人工核对的方法,与计划定位CT图像进行匹配,获取摆位误差并纠正。结果 78例患者行k V-CBCT纠正胸腹部VMAT摆位误差,在X、Y、Z轴方向,差异均有统计学意义(P<0.05)。结论采用k V-CBCT验证胸腹部VMAT可有效提高摆位误差的准确度。

Abstract:Objective The accuracy of clinical setup errors in volumetric modulated arc therapy(VMAT)for treating thoracic and abdominal tumors by kilovoltage cone beam computed tomography(kV-CBCT)was quantified.Methods 78 patients with thoracic and abdominal tumors treated with VMAT in the Department of Radiotherapy in Fujian Medical University Union Hospital from March 2017 to September 2019 were selected in this study.All the patients were treated with kV-CBCT before further treatment and then the real-time three-dimensional CBCT verification images were acquired.Before the setup errors were obtained and corrected,the CBCT images were registrated to the planned CT images by automatic gray registration combined with manual check.Results According to the results of 78 patients treated with thoracic and abdominal VMAT corrected by kV-CBCT,there were significant differences about setup errors in the X,Y,and Z axes(P<0.05).Conclusion The accuracy of setup errors could be effectively improved in treating patients with thoracic and abdominal tumors by kV-CBCT after verified by VMAT.
