原发性高血压分级与高尿酸血症颈总动脉内径之间关系的探讨

摘    要:目的探讨原发性高血压分级与高尿酸血症及颈内动脉内径之间的关系。方法对100例高血压伴高尿酸血症患者,按不同血压水平(1、2、3级)进行分组,分别测量每组的血尿酸(SUA)、胆固醇(TC)、甘油三酯(TG)、高密度脂蛋白(HDL)、低密度脂蛋白(LDL)、载脂蛋白A1(APOA1)、载脂蛋白B(APOB)、颈总动脉内径并进行比较。结果各组SUA水平差异有统计学意义(P<0.01),随血压升高SUA升高;各组血生化比较,TG、TC(P<0.01)以及LDL(P<0.05)差异均有统计学意义,而APOA1、HDL、APOB及颈总动脉内径差异无统计学意义(P>0.05)。两两组间比较,Ⅰ,Ⅱ组间:TG(P<0.05)、LDL(P<0.01)差异有统计学意义;Ⅱ,Ⅲ组间:SUA和颈总动脉内径(P<0.05)差异有统计学意义;Ⅰ,Ⅲ组间SUA、TG和颈总动脉内径(P<0.01)以及TC、LDL(P<0.05)差异均有统计学意义。结论高血压患者SUA水平升高的程度与高血压的临床分级及脂质代谢有关,与颈总动脉内径有相关性,但不密切。

Abstract:Objective To investigate the relationship of hypertension and hyperuricemia and internal diameter of carotid artery. Methods One hundred patients with hypertension and hyperuricemia were divided in groups according to severity of high blood pressure (grades 1,2, and 3). The internal diameter of carotid artery, levels of serum uric acid (SUA), cholesterol (TC), triglyceride (TG), high density lipoprotein (HDL), low density lipoprotein (LDL), apolipoprotein A1 (APOA1), apolipoprotein B(APOB) of the groups were measured and compared among the groups. Results Among the graded groups, significant difference was found in level of SUA (P〈0.01) which increased along with the blood pressure. TG, TC (P〈0.01) and LDL (P〈0.05) also showed significant differences among these groups. The levels of APOA1, HDL and APOB as well as the internal diameter of carotid artery were comparable (P〉0.01). Group pair-wise comparison showed differences in: TG (P〈0.05) and LDL (P〈0.01) between groups I and Ⅱ ; SUA and carotid artery internal diameter (both P〈0.05) between groupsⅡ and Ⅲ; SUA, TG, carotid artery internal diameter(all P〈0.01), TC, LDL (both P〈0. 05) between groups I and Ⅲ. Conclusion The level of serum uric acid in the patients with hypertension was shown to be correlated with clinical grading of hypertension and lipid metabolism. The increase in SUA was also linked but not strongly to the carotid artery internal diameter.
