摘    要:目的探讨下呼吸道感染标本常见革兰阴性杆菌分离株中AmpC酶和超广谱β-内酰腔酶(ESBLs)的检出情况。方法 通过酶提取物三维试验检测单产AmpC酶、单产ESBLs、同时产AmpC酶和ESBLs菌株。结果 280株革兰阴性杆菌耐药株中检出单产AmpC酶菌株43株,以阴沟肠杆菌检出率最高。单产ESBLs菌株134株,以肺炎克雷伯菌检出率最高。同时产AmpC酶和ESBLs菌株19株,以阴沟肠杆菌检出率最高。结论 正确区分ESBLs和AmpC酶,对临床选择性用药具有重要的意义。

Clinical detection of ampC and extendedspectrumβlactamases producing strains in lower respiratory Tract Infections

Abstract:Objective The prevalence of AmpCβ-lactamases and extended-spectrumβ-lactamases (ES- BLs) producing strains were investigated from specimens of lower respiratorY tract. Methods AmpCβ-lactama- ses producing and AmpCβ-lactamases eombinedwith ESBLs producing strains were detected by three-dimensional test. Results Among 280 clinical isolates of Gram-negative bacilli, single birth of AmpCβ-lactamases had 43 strains. E. cloacae had the highest detection rate. ESBLs only had 134 strains, and K. pneumoniae had the highest detection rate. In 19 strains of AmpCβ-lactamases and ESBLs, E. cloacae had the highest detection rate. Conclusions Classifying AmpCβ-lactamases and ESBLs correctly have important significance in clinical chosing medicine.
