原发性双侧乳腺癌的超声诊断价值

目的:探讨超声对原发性双侧乳腺癌的诊断价值。方法:回顾性分析58例经手术病理证实的原发性双侧乳腺癌的超声表现,并以形态、边缘、纵横径比、内部及后方回声、有无钙化和血流丰富程度为观察指标比较双侧乳腺癌在声像图表现上的相似性。结果:原发性双侧乳腺癌的分布部位以外上象限居多,多表现为不规则分叶或伪足状;边缘毛刺征,界限不清;肿块纵横比大于1;内回声光点粗大而不均匀;常见砂粒样钙化灶;后方回声衰减;血流信号丰富。18例(31.0%)双侧乳腺癌的声像图表现具有相似性,而另外40例(69.0%)则不具有相似性。结论:原发性双侧乳腺癌具有类似原发性单侧乳腺癌的二维声像图表现及血流分布特征,第二癌与第一癌相似性较低。

The value of ultrasonography in the diagnosis of primary bilateral breast cancer

Objective:To explore the value of ultrasonography in the diagnosis of bilateral primary breast cancer.Methods:The ultrasonographic manifestations of 58 cases of bilateral primary breast cancer confirmed surgically were reviewed retrospectively,and the second cancer was compared with the first cancer by evaluating the shape,margin,ratio of anteriorposterior in length,inside echoes, rear attenuation,calcification and color flow signals investigate whether the bilateral breast cancers have similar ultrasonographic appearances.Results:Bilateral primary breast cancers were more located in upper outer quadrant,frequently spiculation,with irregular shape,anteriorposterior in ratio length>1,heterogeneous internal echo and acoustic shadowing,containing microcalcification and abundant color flow signals.Of the total 58 lesion pairs,18(31.0%) pairs had similar US characteristics,whereas 40(69.0%) pairs had different US characteristics.Conclusion:Bilateral primary breast cancers have a two dimensional ultrasonographic appearance and vascular distribution similar to that of unilateral primary breast cancers,while the ultrasonographic signs of the first cancer do not indicate the most likely appearance of the second cancer in the contralateral breast.Evaluation of the second cancer should be performed without regard for the ultrasonographic findings for the first cancer.
