腰椎后路术后切口脂肪液化相关危险因素分析

目的探讨腰椎后路术后切口脂肪液化相关危险因素。方法分析387例行腰椎后路手术患者的资料,根据术后切口是否发生脂肪液化分为正常组(355例)和脂肪液化组(32例)。采用logistic回归分析筛选影响术后切口脂肪液化的相关危险因素。结果性别、年龄、术后引流管留置时间两组比较差异均无统计学意义(P>0.05),体重指数(BMI)、糖尿病、高脂血症、多裂肌脂肪浸润程度、手术时间、术中出血量两组比较差异均有统计学意义(P<0.05)。单因素logistic回归分析显示:BMI、糖尿病、高脂血症、多裂肌脂肪浸润程度、手术时间、术中出血量与切口脂肪液化有关(P<0.05)。多因素logistic回归分析显示:BMI、糖尿病、高脂血症、多裂肌脂肪浸润程度、手术时间、术中出血量是术后切口感染的独立危险因素。结论BMI、糖尿病、高脂血症、多裂肌脂肪浸润程度、手术时间、术中出血量是腰椎后路术后切口脂肪液化的独立危险因素。

Analysis of related risk factors of fat liquefection after posterior lumbar surgery

Objective To investigate related risk factors of fat liquefection after posterior lumbar surgery.Methods The clinical data of 387 patients who underwent posterior lumbar surgeries were analyzed.According to whether the wound was liquefied or not,the patients were divided into normal group(355 cases)and liquefection group(32 cases).Logistic regression analysis was used to analyze and screen the related risk factors of influencing postoperative fat liquefaction.Results There were no significant differences in gender,age and postoperative indwelling time of drainage bute between two groups(P>0.05),there were significant differences in body mass index(BMI),diabetes,hyperlipidemia,fat infiltration degree of multifidus,operation time and intraoperative blood loss between two groups(P<0.05).Univariate logistic regression analysis indicated that fat liquefaction were significant correlation with the following risk factors:BMI,diabetes,hyperlipidemia,fat infiltration degree of multifidus,operation time and intraoperative blood loss.Multivariate logistic regression analysis showed that BMI,diabetes,hyperlipidemia,fat infiltration degree of multifidus,operation time and intraoperative blood loss were independent risk factors for postoperative wound infection.Conclusions BMI,diabetes,hyperlipidemia,fat infiltration degree of multifidus,operation time and intraoperative blood loss were independent risk factors for incision fat liquefection after posterior lumbar surgery.
