子宫下段后壁防波堤样缝合术对前置胎盘合并胎盘植入手术的止血效果分析

目的探讨子宫下段后壁防波堤样缝合术对前置胎盘合并胎盘植入手术的止血效果。方法100例前置胎盘合并胎盘植入手术患者,按治疗方法不同分为对照组和研究组,各50例。对照组行常规缝合联合宫颈环扎进行止血,研究组行子宫下段后壁防波堤样缝合术进行止血。比较两组术中出血量、手术时间、产后2及24 h出血量、住院时间,恶露持续时间、子宫复旧时间,新生儿体质量及1、5 min Apgar评分。结果研究组术中出血量及产后2、24 h出血量均少于对照组,手术时间及住院时间均短于对照组,差异具有统计学意义(P<0.05)。两组新生儿体质量及1、5 min Apgar评分比较差异均无统计学意义(P>0.05)。研究组恶露持续时间(40.50±12.50)d、子宫复旧时间(6.20±1.20)个月均短于对照组的(49.35±14.35)d、(7.50±2.30)个月,差异具有统计学意义(P<0.05)。结论前置胎盘合并胎盘植入手术中应用子宫下段后壁防波堤样缝合术的效果显著,能够缩短各项指标时间,减少手术出血量,且不影响新生儿体质量及Apgar评分,值得推广。

Analysis of hemostasis effect of breakwater-like suture of posterior wall of lower uterus on placenta previa with placenta implantation

Objective To discuss the hemostasis effect of breakwater-like suture of posterior wall of lower uterus on placenta previa with placenta implantation.Methods A total of 100 cases of placenta previa with placenta implantation were divided into control group and research group according to different treatment methods,with 50 cases in each group.The control group underwent conventional suture combined with cervical cerclage for hemostasis,and the research group underwent a breakwater-like suture on the posterior wall of the lower uterus for hemostasis.The intraoperative blood loss,operation time,postpartum 2 and 24 h blood loss,hospitalization time,lochia duration,uterine involution time,neonatal body weight and 1-min and 5-min Apgar score were compared between the two groups.Results The intraoperative blood loss,postpartum 2 and 24 h blood loss of the research group were less than those of the control group,and operation time and hospitalization time were shorter than those of the control group.All the difference was statistically significant(P<0.05).There was no statistically significant difference in neonatal body weight and 1-min and 5-min Apgar score between the two groups(P>0.05).The lochia duration(40.50±12.50)d and uterine involution time(6.20±1.20)months of the research group were shorter than(49.35±14.35)d and(7.50±2.30)months of the control group,and the difference was statistically significant(P<0.05).Conclusion The application of the posterior wall breakwater like suture in placenta praevia with placenta implantation can shorten the time of each index,reduce intraoperative blood loss,and do not affect the neonatal body mass and Apgar score,which is worth popularizing.
