格列美脲联合中效胰岛素治疗磺脲类继发失效的研究

目的观察2型糖尿病(T2DM)患者在磺脲类继发失效(SFS)后改用格列美脲联合中效胰岛素(NPH)治疗的临床疗效。方法对63例SFS的T2DM患者,停用原有口服降糖药,改为早餐前服格列美脲,睡前皮下注射NPH,治疗前后测定血糖、糖化血红蛋白(HbA1C)、血脂、胰岛素(Ins)、C-肽(C-P)、体质量指数(BMI)及肝肾功能。结果口服格列美脲联合睡前皮下注射NPH,治疗后空腹血糖(FBG)、餐后2h血糖(2hBG)、HbA1C均有显著性下降(P均<0.01),餐后2hIns、C-P明显上升(P均<0.01)。结论格列美脲联合NPH治疗2型糖尿病磺脲类继发失效,可起到显著改善糖代谢及胰岛β细胞功能的作用。

Effects of Glimepiride and NPH on secondary failure of sulfonylurea

Objective It is to observe the clinical curative effect of Glimepiride and NPH on type 2 diabetic mellitus (T2DM) patients after the secondary failure of sulfonylurea (SFS). Methods 63 SFS patients with T2DM were received Glimepiride once daily and NPH at bedtime. Blood glucose, HbA1C, Insulin (Ins), C-peptide (C-P), body mass index (BMI), liver function and renal function were determined before and after treatment. Results FBG, 2hBG and HbA1C were significantly debased (P<0.01), and 2 h Ins and C-P were significantly risen (P<0.01) after Glimepiride taken orally and NPH subcutaneous injected at bedtime. Conclusion Glimepiride combined with NPH can significantly improved glycometabolism and the beta-cell function in treating SFS patients with T2DM.
