经阴道及腹部B超在无痛高危人工流产术中的临床应用

目的:观察经阴道及腹部B超用于无痛高危人工流产术的临床效果。方法:对2005～2008年292例具高危因素施行无痛人工流产手术患者的临床资料进行分析,术前均经阴道超声诊断,术时采用腹式超声实时监护;并与2000～2003年未行超声监护下的高危人工流产者103例进行对比。结果:292例手术均一次性吸宫成功,未发生吸宫不全、漏吸、子宫穿孔、人工流产综合反应及盆腹腔脏器损伤、宫颈断裂、术中大出血、宫颈或宫腔粘连等并发症,术中出血少。结论:高分辨阴道B超能够早期诊断,提高诊断准确率;在腹式B超监护下行无痛负压吸引术安全有效。

Clinical application of transvaginal ultrasound and transabdominal ultrasound in painless artificial abortion with high risk factors

Objective:To observe the clinical effect of transvaginal ultrasound and transabdominal ultrasound on painless artificial abortion with high risk factors.Methods:The clinical data of 292 cases of painless artificial abortion with high risks factors from 2005 to 2008 were analyzed retrospectively,and all the cases underwent preoperative transvaginal ultrasound and transabdominal ultrasound during operation.The results were compared to 103 cases of painless artificial abortion with high risks factors without ultrasound monitoring.Results:Uterine aspirations in the operations were successful without second try for all the cases,with slight hemorrhage during operation and without complications such as incomplete or leaky uterine aspirations,uterine perforation,artificial abortion syndrome,basin-peritoneal organs injury,cervical fracture,massive hemorrhage during operation and cervical or intrauterine adhesion.Conclusion:High-resolution transvaginal ultrasound can make early diagnosis and improve the diagnostic accuracy.Painless vacuum aspiration under the monitoring of transabdominal ultrasound is effective and safe.
