功能性垂体腺瘤的伽玛刀治疗

目的评价功能性垂体腺瘤伽玛刀治疗的疗效和并发症。方法自1998年8月至2002年8月间对伽玛刀治疗的400例功能性垂体腺瘤病人进行回顾性分析。其中236例随访资料完整,病人年龄在15~60岁之间,均有影像学及临床诊断,其中20例有病理诊断。所有病人均使用伽玛刀治疗,肿瘤直径在5~25mm,平均15mm。伽玛刀治疗处方剂量9~35Gy。结果本组236例获得随访,平均随访24~36个月。肿瘤消失168例(71%)、缩小60例(25%),复发2例,其中激素水平恢复正常190例(80%),临床症状改善212例(90%),病情加重1例(0.42%)。结论伽玛刀治疗功能性垂体腺瘤是一种安全有效的方法,不仅能控制肿瘤,而且能使内分泌功能大部分恢复正常。

Gamma knife treatment of functional pituitary adenoma

Objective To Evaluate the effect and complications of gamma knife treatment for functional pituitary adenoma.Methods Between August 1998 and August 2002,400 Cases of Functional Pituitary Adenoma were treated by gamma knife and follow-up was carried out.236 cases have complete follow-up data.Patients ages between 15 and 60.All patients had image and clinical diagnosis,among which 20 had pathologic diagnosis.Tumor sizes range from 5 to 25 mm,average 15 mm.Prescribed dose was 9 to 35 Gy.Results During 24 and 36 months,tumor disappearance was in 168 cases(71%),size reduction in 60 cases(25%),recurrence in 2 cases.Hormone level returning normal in 190 cases(80%),clinical symptom improvement in 212 cases(90%),deterioration in 1 case(0.42%).Conclusion Gamma knife treaument is used in functional pituitary adenoma not only kills tumor,but also brings incretion to normal.Care to be taken not to harm normal pituitary.
