细胞因子与急性胰腺炎病理变化之间关系的研究

目的：了解细胞因子水平与急性胰腺炎病理变化之间的关系。方法：采用ELISA法检测发病36小时以内的急性轻症胰腺炎组和急性重症胰腺炎组血浆TNF-α和IL-6水平以及急性重症胰腺炎组胰性腹水中TNF-α和IL-6水平。结果：急性轻症胰腺炎组和急性重症胰腺炎组的血浆TNF-α和IL-6水平均显著高于对照组（均P＜0.01)，重症胰腺炎组TNF-α和IL-6水平又显著高于对照组（P＜0.01)，重症胰腺炎组TNF-α和IL-6水平显著低于其血浆水平（P＜0.01)，与血浆轻症组比较无显著差异（P＞0.05)，但显著高于血浆对照组(P＜0.01)。结论：血浆细胞因子水平与急性胰腺炎病理变化程度之间关系非常密切，胰性腹水细胞因子水平至少在发病早期对判定急性胰腺炎病理变化程度具有参考价值。

The Relationship between Cellular Factor and Pathologic Changes of Acute Pancreati tis

Objectives: To investigate the relationship between cellular factor and pathologic changes of acute pancreatitis. Methods: The levels of TNF-a and IL-6 in plasma were detected in patients with acute pancreatitis and the patients were divided into two groups: mild pancreatitis and severe pancreatitis. The levels of cellular factor of pancreatic ascites in severe pancreatitis were also detected. All samples were collected during the first 36 hours of acute pancreatitis. Results: The levels of TNF-a and 1L-6 of plasma in mild pancreatitis and severe pancreatitis were all significantly higher than those of control group (all P＜0. 01),meanwhile the levels of cellular factor in severe pancreatitis was significantly higher than that of mild pancreatitis (P＜0. 01). The levels of TNF-a and II-6 of pancreatic asciles in severe pancreatitis were significantly lower than that of its plasma (P＜0. 01),and no significance can be found as compared with that of plasma in mild pancreatitis. But still significantly higher than that of plasma in control group (P＜0. 01). Conclusions: The plasmic levels of cellular factor have a close relationship with the severity of acute pancreatitis. The levels of cellular factor of pancreatic ascites have their reference value at least in the early pathologic stage of acute pancreatitis.
